We ask that you complete all forms and questionnaires on the patient portal at least 48 hours prior to your appointment. You may also upload recent test results that you would like us to review.

Getting Started

In short, no. Due to the nature of extensive care in this practice, we do not accept any form of medical insurance, including Medicare and Medicaid.

We have opted out of Medicare. Patients with Medicare or Medicaid (even if it is secondary) cannot submit for reimbursement. If you are enrolled in Medicare Part B, we ask that you fill out a Medicare opt-out form.

To simplify the process, we recommend calling your insurance company prior to your visit to find out about what your specific plan will cover. We’re more than happy to provide insurance codes for registered new patients.

After you complete and. submit payment for your visit, we will upload a copy of your superbill to your patient portal. It will contain the appropriate CPT (Current Procedural Terminology) and diagnostic codes. This is what you will send to your insurance provider. Since each individual’s insurance coverage varies, we encourage you to contact your insurance provider ahead of time and ask about their reimbursement rates and policy. We cannot guarantee reimbursement.

We have found that some insurance companies request additional information that is very specific and specialized to that provider. Please note that given the large number of discrepancies between what each company requires and that time needed to meet those requests, we do not assist with insurance reimbursement above and beyond the diagnosis, tests codes, and receipt given at check out. All codes used and applied to these superbills are a direct reflection of your consultation and will not be changed or removed.

While we strive to provide value, care and assistance to our patients, our office is not responsible for calling and negotiating EOBs (Explanation Of Benefits), making individual changes to bills to support an insurance provider’s requests, or disputing coverage for any particular codes used.

Patients may be able to utilize their flexible spending account or health savings account to pay for their visits. Please contact your insurance provider to confirm. If payment is denied, the patient is responsible for payment of services rendered.

Any cancellations or rescheduling requests must be made at least 72 hours prior to your appointment date and time to avoid a cancellation charge equal to the full cost of the appointment.
Financial Policy

We accept cash, check, and all major credit cards. We ask that our patients pay the full balance when services are rendered.

The balance is due at the conclusion of your visit.

We ask that you share your most updated credit card information securely on the patient portal. This allows us to charge the card on file immediately after each visit to ensure a seamless process for each patient. We also require a payment method on file in the case that cancellation fees are applied.

Decline or missed payments have 3 business days to be processed correctly. If payment continues to be declined or is unable to be processed, we will place a hold on all future appointments and services. Please ensure that all balances are paid in a timely fashion to avoid disruption in your treatment plan.

We understand that the costs of our services are comparatively higher than many other providers. We do not offer payments plans. Functional and integrative medicine is a comprehensive, ongoing process, therefore we want to ensure that every patient who begins this journey with us is prepared for the full scope of that investment, both financially and personally. We're happy to walk you through expected costs during your initial consultation so there are no surprises.
Payment Options

Conventional labs are those that you are likely already familiar with. These are the labs such as thyroid testing, liver function testing, and lipid panels, to name a few.

We are different from your conventional doctor in that she orders Functional Medicine lab tests and interprets them differently than a conventional doctor would. We listen to your symptoms while looking at optimal levels rather than normal ranges.

Our practice uses high-quality functional lab tests to uncover the root causes of illness. Many tests are covered by insurance, though some may not be — we recommend contacting your provider before completing any labs to confirm coverage. Please note that certain plans, such as Medicaid, do not cover tests ordered by out-of-network providers.
At checkout, we'll provide the necessary paperwork and billing codes for you to submit for reimbursement. HSA and FSA funds may also be accepted.

You are welcome to share your past medical records, including any lab studies and imaging. We will only begin to order labs during your initial consultation.

Your lab requisition form will be added to your portal under the “Labs & Documents” tab. Please bring the printed document with you to your lab appointment.

We typically refer patients to LabCorp, Quest Diagnostics, or BioReference Laboratories. Your insurance provider may prefer a certain lab company. We encourage you to contact your insurance provider to determine which lab they prefer.

Your final lab results will be posted to your portal when ready. You can located them via the “Labs & Document” tab on the portal.

All test results must be reviewed during an appointment. Once you have your labs done, please let us know so that we can set up your follow-up appointment to review the results.
